The number most people track is the wrong one.

When I got my baseline testosterone panel, my total T came in at 599 ng/dL. Not bad. Not exceptional. Pretty average for a man in his late 30s who trains regularly and takes his health seriously.

Three months later, my total T had dropped to 537 ng/dL. On paper, that looks like a failure.

But my free testosterone β€” the fraction your body can actually use β€” went from 93.0 pg/mL to 123.47 pg/mL. That's a 32.8% increase.

Why free testosterone matters more than total

Total testosterone measures everything in your blood β€” bound and unbound. But roughly 97–99% of testosterone in your blood is bound to proteins, primarily SHBG and albumin. Bound testosterone can't activate androgen receptors. It can't build muscle, drive libido, or support cognitive function.

Free testosterone is the active fraction. It's what your cells can actually use.

When SHBG rises, it captures more testosterone β€” total T looks stable or even high, but free T drops and you feel it. When SHBG falls, more testosterone becomes bioavailable even if total T stays flat.

The goal of any testosterone optimization protocol, in my view, isn't to maximize total T. It's to maximize free and bioavailable T while keeping estradiol in a healthy range and SHBG manageable.

The protocol

What I take from this

Total testosterone dropped 10%. I'm not concerned by that β€” total T fluctuates significantly based on sleep, stress, time of day, and hydration. A single draw is not a reliable trend signal.

What I do find meaningful: free testosterone increased consistently across both draws. Month over month. That's not noise.

SHBG behavior was interesting β€” it rose in month 2 then fell below baseline by month 3. Several compounds in this stack are associated with SHBG modulation, including shilajit's fulvic acid fraction and boron-adjacent mineral activity, as well as tongkat ali. I'm not attributing cause and effect here. I'm reporting what I measured.

The cardiovascular and metabolic markers are where I'd expect anyone's attention to go. LDL down 6%. Triglycerides down 8%. Trig/HDL ratio at 0.92 β€” well below the 2.0 threshold associated with insulin resistance risk. hsCRP undetectable at both draws. ApoB stable at 70.3 mg/dL.

Thyroid markers shifted notably β€” TSH dropped 44% and Free T3 rose 17% from month 2 to month 3. I added B12 after the month 2 draw which may have contributed. I note it without conclusion.

Vitamin D dropped 15% β€” from 57.3 to 48.3 ng/mL. I'll be increasing D3 intake heading into winter.

Biological age score of 29 years 3 months at the time of the month 3 draw. I'll let that speak for itself.
